Patients should call their LHP physician's office directly to request a prescription refill when there is a week supply still in the bottle. Patients should leave the following information on the voice mailbox:
- Date of birth
- Phone number
- Physician's name
- Medication name and strength
- Pharmacy name and telephone number
Patients will be contacted if there is a problem with the refill request. Please allow 48 hours for prescription refills.
Pain medications will not be refilled without an office visit.
