For Patients
Lutheran Health Physicians patients and guests can expect confidential, professional service from knowledgeable and friendly staff members. The patient-registration staff will get you registered as quickly as possible, and they will make sure your insurance information is filed correctly to ensure proper billing.
New and Established Patients
New patients should plan to arrive approximately 30 minutes before the scheduled appointment to ensure accurate information is obtained to eliminate potential billing problems.
New patients should bring to the first appointment:
- Photo ID
- All your demographic information, such as address, age, etc.
- Emergency contact information
- Current insurance cards
- List of medications and dosages, or actual bottles
- List of any medications to which you are allergic or have had adverse reactions
- List of past surgeries with dates/hospitalizations and lab and/or X-ray results, as well as family history; a complete history and physical will be recorded if you are a new patient.
- Primary care physician's name, address and telephone number
- Completed new patient paperwork, if received ahead of time in the mail. If paperwork was not received in the mail, new patients will need to complete the paperwork prior to seeing the doctor.
Established patients should:
- Arrive 15 minutes before the scheduled appointment and check in at the front desk
- Bring a photo ID and insurance cards to each appointment
- Verify address and phone number at each visit
Arriving Late
Patients who are more than 10 minutes late for an appointment may need to be rescheduled.
Rescheduling
Lutheran Health Physicians asks patients to call the office at least 24 hours in advance if they cannot keep an appointment. Patients will be asked to reschedule at that time. Please help Lutheran Health Physicians offer timely appointments by adhering to the 24-hour notice cancellation policy.
No Shows
Patients are considered "no shows" if they miss an appointment without notice. To reduce the number of lost appointments due to no shows, Lutheran Health Physicians's policy requires a $20 fee for each missed appointment without a 24-hour notice. After three no shows in a 24-month period, patients may be dismissed from the practice.
Minors
Minors (under the age of 18) who are not accompanied by a parent or legal guardian must have a parental preauthorization for medical care to children form completed for services to be rendered.
Medical Records
To share protected health information with another person, an authorization for release of protected health information form must be signed and dated by the patient or a legal representative and a witness. The form must state to whom the records are going and which parts of the medical record are to be released. Patients must be at least 18 years of age to sign a release form. Spouses cannot sign for each other.
After Hours Needs
Fort Wayne Patients
Calls made to (260) 432-2297 after 5 p.m. and before 8 a.m. are automatically transferred to the after-hours answering service. An LHP provider is available 24/7 to assist with urgent needs. In a medical emergency, please call 911.
Warsaw Patients
If you believe your problem is life threatening, go directly to the emergency department or dial 911. After office hours, you may reach the physician on call for urgent care needs by contacting the physician answering service at (888) 442-1044. The operator will ask the reason for your call. A physician will return your telephone call.
Reserve your calls for medical care needs that cannot wait until normal office hours. Do not call the answering service for medication refills, test results or to schedule an appointment. Pain medications will not be refilled during the weekend.
Work/School Forms
Patients must be seen by the physician to obtain a work or school slip to document disability or absence. Excuses will not be given for days of work or school missed prior to your office visit.
In general, an appointment will need to be scheduled for the completion of forms such as FMLA, etc.
